The survey gives school leaders a better understanding of what you and other members of your school community think about your school. The information collected by the survey is designed to support a dialogue among all members of your school community about how to make your school a better place to learn.
For the first-time ever, you can login to your NYC Schools Account to take the parent/guardian NYC School Survey without needing an access code!

Make sure to do the following when completing your paper survey.
Use black ink, blue ink, or pencil.
Fill in the selected answer circle completely.
Do not write over the QR Code.
Do not tear the paper or attempt to remove the staple.
You can return the paper survey by mail using the prepaid green envelope. The survey is also available to take online at www.NYCSchoolSurvey.org
The deadline to complete your NYC School Survey is March 25, 2022.
